The Concord School-Linked Health Center is proud to announce that the Blue Cross Blue Shield Foundation has provided the Center with funds to purchase all vaccines that are recommended and mandatory for school attendance for children K-12. The Center is excited to expand their healthcare services to the greater Concord community and the surrounding school districts. Please call and schedule an appointment at 517-750-7393. Tuesday, October 8, 2019, there will be a speaker coming to Polling Center on the Spring Arbor Campus, to share his story about his son who died from a Heroin overdose, and talk about FAN ( Families against Narcotics). This event is being sponsored by the Concord School-Linked Health Center and presented by the Social Work Department at Spring Arbor University. The presentation is from 6:00 p.m. until 7:00 p.m., and is open to faculty, students, and the community.
On Tuesday, July 30th. 2019, The Concord School-Linked Health Center will be doing free Sports Physicals for Concord Athletes from 8:30 a.m. to 4:30 p.m. All other non Concord Athletes will be charged $30. Your name will be put in for a drawing for Tigers Tickets. It will be a first come first serve basis, no appointments. Please have the Michigan High School Athletic Association, INC Physical Exam & Clearance & Consent Form completed and signed by the parents. This will allow us to do the Physical without the parent present. The forms are available from the Concord School Administrative Office, The Center, Coaches, or Athletic Director.
On Saturday, June 22, 2019, we will have S.A.F.E. Archery activity for all ages. This event will be located on the corner of Main and Homer Street. The cost will be $3 for six arrows. There will be people there to assist with the activity. Looking forward to seeing you there for some great fun!
Sports Physicals will be provided on a walk-in basis after April 15, 2020, for the next 2020-2021 school year. The Michigan High School Athletic Association, Inc. Physical Exam & Clearance & Consent Forms are available at the school office, athletic director, coaches, and from the Center. Please have forms completed and signed by the parent. The cost for Sports Physicals is $30.
